The west coast has the rail and coach spine, the food cities and the drier weather for most of the year, and skipping Borneo keeps the twelve days from turning into a sequence of airports. The budget shown is a mid-range figure for the twelve days on the ground, excluding international flights, and it drops sharply if you sleep in dorms and eat from stalls.
The only bookings that need to be early are the ETS train seats around a weekend, a room in the Cameron Highlands during school holidays, and the Langkawi flight if you are travelling between December and February. Everything else can be arranged a day or two ahead once you see what the weather and your appetite for moving are doing.

Arrive in Kuala Lumpur
Take the express train in from the airport and keep the first evening to a short walk and a plate on Jalan Alor.

Chinatown and the old centre
Walk Petaling Street, Merdeka Square and the river mosque before the heat, then take an air-conditioned museum in the afternoon.

Batu Caves and the towers
Ride the Komuter line north early for the caves, then use the evening for the Petronas skybridge slot you booked ahead.

Coach up to the Cameron Highlands
Four hours of switchbacks to Tanah Rata, arriving to cool air and a steamboat dinner rather than another sight.

Tea estate and mossy forest
Walk into the BOH estate in the morning, then take the guided road up to the mossy forest if the ridge is clear.

Travel to George Town
Come down the hill and cross to Penang by coach, leaving the afternoon free rather than stacking a sight on arrival.

The heritage core on foot
Walk the clan jetties, Khoo Kongsi and the shophouse lanes early, then eat your way along one hawker street after dark.

Penang Hill and Kek Lok Si
Take the funicular up for the cooler air and the view, and stop at the temple at the foot of the hill on the way back.

Keep this one loose for the museums, a cooking class or the streets you walked past the first time.

Cross to the island, pick up a small rental car and settle on the west coast in time for the sunset.
