Three days in the capital and three in George Town use the west-coast train, and a single flight east buys the part of Malaysia you cannot see on the peninsula. Flying home from Kota Kinabalu instead of returning to Kuala Lumpur saves a full day, so price an open-jaw ticket before you book a return.
Book in this order: the Kinabatangan lodge first, then the domestic flights around it, then the rooms. Two nights on the river is the useful minimum, the transfers are usually part of the lodge package, and no operator can promise you an orangutan. The budget shown is a mid-range figure for sixteen days on the ground, excluding international flights, and the Borneo half accounts for most of it.

Take the express train in from the airport and keep the first evening short, with one hawker street and an early night.

The old centre on foot
Walk Chinatown, Merdeka Square and the river mosque in the morning, then take the Islamic Arts Museum in the afternoon heat.

Batu Caves and Bukit Bintang
Ride north early for the caves, then spend the evening between the towers and the food street below them.

ETS train north to Penang
Four hours up the west coast to Butterworth and across to George Town, with the afternoon left deliberately empty.

Walk the clan jetties and shophouse lanes before ten, rest through the middle of the day and eat late.

Take the funicular for the cooler air, then work through one hawker centre properly instead of three in a hurry.
